Common Forms:

Practice Logs - To be signed by parent/guardian each week and submitted the Director. Each instrumental program uses this same form.
Private Lesson Forms - Students taking private lessons should complete this form. The form needs to be signed by the student, instructor and parent/guradian, then submitted to the director prior to the end of each marking period.
Website Consent Form - Most of the photographs posted on this website are of groups taken from a distance, but sometimes pictures of smaller groups or individuals may be posted. This website is intended to be informational and to show our pride in students who take part of the various music and guard programs. It is not our intent to cause undue concern by posting recognizable photographs. Therefore, we ask parents and guardians to complete this consent form and submit it to the directors or executive board members.
Authorization for Medicine Use at School - Our organization adheres to school policy concerning administration of any medicines to students. That policy requires that this form be completed, signed by a physician and on file with the school. Please submit this form to the director as needed.
Volunteer Form - With both general volunteer activities throughout the year and specific program activities, we can always use help. Complete this form and turn it in to our president, Paul Gentine, or any of the executive board members.

Marching Band Forms/Information:

Orientation Handbook - Provides general information about participation in the Flushing Raider Marching Band and the Flushing Raider Band & Orchestra Boosters including band camp and competition information.
Marching Band Intent - Student completes this form indicating intent to be a member of the Flushing Raider Marching Band.
Uniform Pieces - Used for ordering pieces of the uniform such as dinkles (marching band shoes), shirts, gloves and jackets.
